DragonFly and Athletic Forms

Please go to DragonFly MAX (dragonflyathletics.com) to register and upload sports forms.  All K-8 physical and medical forms required for sports participation are on the Dragonfly site.  We no longer accept paper forms for sports participation.  Our school code is KEZTY4.

For K-5 recreational sports, payment is due at the time of sign up.  For middle school sports, payment is due once the coach sends out the official team roster.  There is no cost to try out.  You only need to pay if you are given a roster spot on the team.
