Health Room (Forms)

St. Mark Catholic School has a full-time health room.  Kathleen Herman, MSN, and Megan Harris, RN, are our two nurses on staff, each working 2 to 3 days per week.  

In order for our nurses to administer any medication at school (even Tylenol or Advil), you must have a medication authorization form on file.  If your child has an allergy, asthma, diabetes, or seizures please be sure to note it and complete the appropriate action plan.
